Since 2010, all people with long-terms conditions have had to be offered the opportunity to make a care plan. This could include recording information on medication, setting goals to improve health and wellbeing or putting plans in place should their health condition get worse.

It has been shown that, amongst high risk groups, GP visits reduce by 40 per cent and hospital admissions by 50 per cent, when people make a personal care plan.

The government is keen that the NHS achieves a significant increase in the use of technology to help people manage their health and care and expects that by March 2015 everyone who wishes will be able to get online access to their own health records held by their GP.
