We are consulting with local authorities on how health and social care systems can be joined up to deliver on Better Care Fund and Digital First objectives.
We are working to develop a new model of care provider, which helps to keep people well in their homes and communities using co-commissioned health and social care models.
Our vision is for city-wide Access hubs which house multi-disciplinary teams from primary care, community, mental health, local authority and acute organisations, providing one point of contact for citizens for their health and social care needs outside hospital.
Supported by our products like DLS Care Planning and DLS Connect, this model encourages citizens to self-manage and creates a test bed for integrating care around the citizen. As part of this consultation we are also able to provide bid support to obtain UK and European transformation funds.
Joined-up city-wide health and social care provision has the potential to make services more personalised for citizens, encourage self-management, help share information and make service provision more efficient using everyday citizen-owned technology.
